If you use insurance, many services require pre-authorization--particularly psychological testing.
Please contact Dr. Bailey Arizpe for her new patient forms. Once you have completed the forms and sent them back to her, then she forwards them to her biller. Her biller calls to get authorization for services. Once authorization is obtained, then an appointment is scheduled.
Things to remember about insurance:
- You may have a co-pay for services.
- You may have an unmet deductible--services will be out of pocket until that deductible is met.
- Your insurance may only allow so many sessions per calendar year.
- Your insurance may require the YOU get authorization such as EAP sessions.
